We are taking a limited number of enrolments for our Pilot Program running from Saturday 5 October to Friday 20 December 2019.   PLACES ARE LIMITED.

If you want to enrol, please complete and submit this form. We will contact you within 3 working days to advise you of your reserved place in the Program or otherwise.

By completing this form, please understand we are collecting your details for the purpose of contacting you about becoming a participant in the MOVE YOU Program.  Also, we will use these details to confirm your eligibility to participate with your GP / Medical Centre.  These details will be available to persons engaged by the MOVE YOU providers for that purpose.  At any time, you can ask us to access and/or correct your details by contacting us at support@moveyou.co.nz
