We have received your Request for Enrolment.
An email should be on its way to your inbox (please check your promotions and junk folders if you do not receive it in the next few minutes).
If you are a patient of Silverstream Health Centre, we will now check your eligibility for admission to the Program and funding with your GP. If you are not a patient of Silverstream Health Centre, we will in touch with you directly.
We will come back to you within 3 working days to confirm your reserved place in the Program or otherwise.
Once your place is reserved, you will need to:

ensure you have your blood test and urine sample collected by Friday 4th October 2019 (We will send you more information about this once you are confirmed in the Program); and

pay your portion of the Program Fee ($100.00 if you are a patient of Silverstream Health Centre).
We will let you know the account details when we confirm your reserved place on the Program.
